To register as a new patient at My Health you will need to complete a New Patient Questionnaire for each patient wishing to register and hand these to one of our reception teams to be processed. Please bring ID with you when dropping in your completed forms. We cannot see you until this process has been undertaken (Please refer to our registration policy below and why we ask for this).
Acceptance on to the list is as at the discretion of the Doctor. Please note that your medical records may take several weeks to arrive at the Practice.
We will accept patients beyond our boundary, if this is clinically appropriate. However, we are unable to provide home visits to those patients.
There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.
